Strengthen Partnerships to Improve Access to Care
Access to healthcare services remains a significant challenge for many individuals in our community, particularly those who are uninsured, underinsured, face transportation barriers, experience language barriers, or live in areas with limited provider availability.
Beacon Community Impact’s Healthy Community priority focuses on improving access to care through stronger community partnerships, enhanced healthcare navigation, workforce development, and expanded access to providers and services. Through collaboration with community organizations, federally qualified health centers, schools, and healthcare providers, these efforts seek to reduce barriers to care and ensure residents can access the services they need to achieve optimal health outcomes.
Without targeted efforts to address these barriers, many community residents will continue to face challenges accessing timely and appropriate healthcare services. That is why Beacon Community Impact supports initiatives that strengthen healthcare access, improve care navigation, expand provider availability, and connect individuals with the resources they need to achieve better health outcomes.
As part of Beacon Community Impact’s response there are three outcome measures we will focus on:
Outcome 1
Increase % of social needs met for community residents and Beacon patients.
Outcome 2
Decrease hospital readmission rates for vulnerable populations.
Outcome 3
Decrease in individuals using the ED for non-emergency conditions.
