Every three years, Beacon Community Impact completes a Community Health Needs Assessment (CHNA) to understand the greatest needs of the communities in Beacon Health’s service area. The 2025 CHNA report details these qualitative and quantitative findings from the community.

Improve access to providers

Expand access to non-clinical community health services by integrating community health workers, peer support specialists, peer recovery coaches, and health navigators across healthcare and community settings to address social drivers of health, improve care coordination, strengthen care transitions, reduce barriers to care, and connect individuals and families with resources that support long-term health and well-being.

Through collaboration among healthcare providers, community-based organizations, social service agencies, and public health partners, establish sustainable pathways that improve access to supportive services, advance health equity, increase patient engagement, and ensure individuals receive coordinated, culturally responsive support that meets their health and social needs.

Recognizing the significant impact of social drivers of health on health outcomes, Beacon Community Impact has placed an emphasis on strategies that improve access to non-clinical community health services:

Outcome 1

Increase % of social needs met for community residents and Beacon patients.  

Outcome 2 & 3

Decrease hospital readmission rates for vulnerable populations.  

Decrease ED visits for vulnerable populations.  

Outcome 4

Improvement in maternal and infant health outcomes (high blood pressure, pre eclampsia, maternal mental health) in historically underserved populations.
